The National 3D Prevention Month Coalition is interested in obtaining information about 3D Prevention Month activities conducted by recipients of the 1997 Program Planner. Please complete this Brag Sheet so that we can learn firsthand about activities taking place throughout the country.

Three grants of $500 each will be awarded via random selection by the Coalition to thank you for taking the time to complete this form. Note that the Planner Feedback portion of the form must be completed in order to be eligible for a grant.

The National Highway Traffic Safety Administration (NHTSA) will publish an issue of the 3D Exchange in Spring 1998. Articles will be written from Brag Sheet submissions. Innovative programs will receive national attention and publicity if published.
